Business Forms & Applications

Welcome CFCU members! The information below will help you to transition your account to a Virginia Credit Union business account. 

Here is a list of forms and documents you’ll need to transition your business account. You can fill, print, sign and return the forms in the envelope provided in your business packet, or you can return them to any CFCU branch. We’ll need to receive your completed business membership package and all other documents by May 20, 2020 to avoid interruption in service. 

Business Membership Package

All forms listed below must be filled out completely, printed and signed. We are unable to process incomplete applications.


In addition to the Business Membership Package, each of the following types of businesses require additional documentation.

Sole Proprietorship/DBA

  • Completed and signed Business Membership Package
  • Employer Identification Number (EIN) confirmation letter from the IRS
  • Business License, if available
  • Certificate of Fictitious/Assumed name, if applicable

Partnership

  • Completed and signed Business Membership Package
  • Partnership agreement
  • Employer Identification Number (EIN) from the IRS
  • Business License, if available
  • Copy of the Certificate of Partnership, if applicable
  • Copy of the SCC registration, if registered with the SCC
  • Certificate of Fictitious/Assumed name, if applicable

Unincorporated Organization

  • Completed and signed Business Membership Package
  • Articles of Organization
  • Minutes of last meeting signed by the Association/Organization’s officer(s)
  • Employer Identification Number (EIN) from the IRS
  • Authorization to act on behalf of Association/Organization

Corporation

  • Completed and signed Business Membership Package
  • Articles of Incorporation
  • Copy of the SCC registration
  • Employer Identification Number (EIN) from the IRS
  • Bylaws, if available
  • Business License, if available
  • Certificate of Fictitious/Assumed name, if applicable

Limited Liability Company (LLC)

  • Completed and signed Business Membership Package
  • Articles of Organization or Operating Agreement
  • Copy of the SCC registration
  • Employer Identification Number (EIN) from the IRS
  • Business License, if available
  • Certificate of Fictitious/Assumed name, if applicable
